DEPARTMENT: PHARMACY

JOB TITLE: STAFF PHARMACIST II

REPORT TO: DIRECTOR OF PHARMACY

BASIC FUNCTION: Under the general direction of the Pharmacy Manager, the Staff Pharmacist II is responsible for the day to day operations of the pharmacy including dispensing and consulting on prescriptions, financial performance, compliance with state and 340B regulations, staff performance, and customer service.

REPRESENTATIVE DUTIES: The duties recorded below are intended to accurately represent the duties of the class and are not intended to cover every single duty of the job.

A.

· The pharmacist receives a new prescription order from the prescriber, consults with the patient, identifies, evaluates and interprets a prescription, interprets the clinical data in a patient medication record, consults with any prescriber, nurse, health professional or agent thereof, supervises the packaging of drugs, checks the packaging procedure and product upon completion, is responsible for all activities of pharmacy technicians to ensure that all such activities are performed completely, safely and without risk of harm to patients, performs any other duty which federal or state law or regulation authorizes only a registered pharmacist to perform and performs all functions which require professional judgment.

· Pharmacist transmits a valid prescription to another pharmacist

· Pharmacist provides consultation, training, and education to patients about drug therapy disease management and disease prevention.

· Pharmacist provides professional information and participates in multidiscipline review of patient progress.

· Checks CURES database and processes controlled Rx per protocol.

· Assists in Clinic Medication Orders

· Assists in 340B Compliance Tasks and Audits.

· Complies with 340B regulations and operating procedures

· Assists in State Board of Pharmacy Inspections.

· Other duties as assigned from leads/supervisors/upper management

B.

· Pharmacist administers drugs and biological products ordered by the prescriber

· Pharmacist manufactures, measures, fits to the patient or sells and repairs dangerous devices or furnishes instructions to the patient or patient representative concerning the use of the dangerous devices.

· Pharmacist furnishes medication including emergency contraception drug therapy and self-administered hormonal contraceptives, nicotine replacement products, prescription medication not requiring a diagnosis recommended by the Centers for Disease Control when traveling outside of the US

· Pharmacist administers immunizations pursuant to a protocol; orders and interprets tests for monitoring and managing efficacy and toxicity of drug therapies.

· The pharmacist as part of the care provided by a health care facility, a licensed clinic and a licensed home health agency in which there is physician oversight, or a provider who contracts with a licensed health care service plan with regard to the care or services provided to the enrollees of that health care service plan, is performing the following functions, in accordance with policies, procedures, or protocols of that facility, licensed clinic, or health care service plan that were developed by health professionals, including physicians and surgeons, pharmacists and registered nurses. The functions are: ordering or performing routine drug therapy related patient assessment procedures; ordering drug therapy related laboratory tests; administering drugs or biologicals by injection; initiating and adjusting the drug regimen of a patient; and performing moderate or waived laboratory tests.

· Assists in Clinical Pharmacy initiatives such as MTM’s.

· Assists in Med-Sync Program.

· Assists in optimizing inventory by maintaining accurate inventory levels, optimizing inventory purchases, and utilizing our pharmacy inventory software in our pharmacy system.

· Assists in completing scheduled pharmacy tasks that are to be done on a daily, weekly, monthly, quarterly, and annual basis.

· Ability to document processes and follow them.

· Performs Medication Reconciliation reviews on patients’ profile

EDUCATION AND EXPERIENCE:

· Active Pharmacist License from the State of California.

· Completion of 340B Certification Program. (Can complete within 1 year of starting)

KNOWLEDGE AND ABILITIES:

· Meets operational standards including performance, safety, and compliance.

· Able to learn and follow established procedures, rules, and regulations.

· Computer skills as necessary to operate pharmacy software, electronic medical records, email, excel, and other necessary software programs.

· Adheres to work schedule: attendance, punctuality, time management.

· Ability to communicate in a courteous and professional manner. Can effectively communicate with health care providers, vendors, patients, and coworkers.

· Works effectively within a team. Is open to feedback and offers feedback when necessary.

· Provides a welcoming environment to our patients. Is attentive to patients needs and displays a positive attitude.

· Demonstrates understanding of 340B Compliance.

· Keeps up to date on pharmaceutical knowledge to be able to effectively consult our patients and health care practitioners.

WORK DIRECTION, LEAD AND SUPERVISORY RESPONSIBILITIES: Supervises Pharmacy Technicians, Pharmacy Clerks, Pharmacy Delivery Drivers, in medication distribution system in accordance with State Laws

PHYSICAL EFFORT: Work requires the ability to stand for long periods of time. Bending, stooping and frequent lifting, constantly up to 50 pounds, to stock medication.

CONTACTS: Supervisor, co-workers, pharmacist provider, medical providers, other Innercare personnel, patients, vendors, and the general public.

WORKING CONDITIONS: Workers risks exposure to odors, communicable disease, toxic substances. Work is performed under stressful conditions. Must comply with the Innercare Corporate Compliance Program.

Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes.

Comply with all regulations regarding corporate integrity and security obligations. Report unethical, fraudulent or unlawful behavior or activity.

Requirements
  • Bachelor's Degree in Pharmacy or Pharm.D
  • Must have an active pharmacy license in state(s) of practice with all held licenses in good standing
Salary Description
$80 / hour
